HIPAA Notice of Privacy Practices
Your rights and our responsibilities regarding protected health information.
Effective date: October 9, 2026
THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.
Our responsibilities
Adapt Clinical, the solo practice of Dr. Monique Vorous, Psy.D., is required, when HIPAA applies, to maintain the privacy and security of your protected health information (PHI), provide this notice of legal duties and privacy practices, follow the notice currently in effect, and notify affected individuals of breaches of unsecured PHI as required by law. California law may provide additional protections.
How your information may be used or disclosed
Treatment
We may use and disclose your PHI to provide and coordinate treatment, including communicating with other treating providers as permitted by law.
Payment
We may use and disclose PHI to bill for services, verify coverage, and obtain payment, including through health plans and billing partners as permitted.
Health care operations
We may use and disclose PHI for lawful practice operations, such as quality assessment, administrative work, compliance, and professional consultation, subject to applicable confidentiality requirements.
Other uses and disclosures permitted or required by law
In limited circumstances, PHI may be disclosed for purposes such as public health reporting, oversight, legally mandated reporting, certain judicial or administrative proceedings, law enforcement requests that meet legal requirements, serious threats to health or safety, or other circumstances authorized or required by law. Additional California confidentiality rules may restrict disclosures.
Uses requiring your authorization
We generally will obtain your written authorization for uses or disclosures not otherwise permitted or required by law, including most marketing uses, sales of PHI, and disclosures of psychotherapy notes as defined by HIPAA, subject to specific legal exceptions. You may revoke an authorization in writing, except to the extent we have already acted on it.
Special protections
Psychotherapy notes maintained separately from the clinical record have special protections. Certain records and information may also receive additional protections under California or other federal laws.
How care, records, and requests are handled
Adapt Clinical is a solo telehealth psychotherapy practice. Sessions are conducted using Zoom and are not recorded. Dr. Vorous writes her own clinical notes without AI transcription or automated note-taking. Clinical records and insurance billing are managed through Headway or Grow Therapy, as applicable. These service arrangements do not eliminate Adapt Clinical’s applicable privacy or patient-access responsibilities.
Requests for records
To request access to or a copy of your clinical records, contact Dr. Vorous at DrVorous@Adapt-Clinical.com or 530-392-8072. Dr. Vorous will coordinate with the applicable Headway or Grow Therapy process to facilitate the request, subject to applicable legal requirements and timelines.
